Please follow this two step process in order to complete the necessary documentation before your first appointment.

Step One

Read these first two documents. These are to be fully reviewed before moving on to the next step:

• Notice of Privacy Practices (2.57 MB)

Step Two

Complete the following four documents. These are to be filled in and brought with you when you visit our office:

• Patient Health Questionnaire
• Informed Consent for Dental Treatment
• Patient Record of Disclosures
• Patient Acknowledgment of Receipt of Dental Materials Fact Sheet
• Patient Policy Consent
